Are you receiving a Home Care Package or NDIS funding?

You must be on a low income to be eligible for this program.

To prove this you need to hold a current:

- Health care card

- Pension card

- Veterans affairs card (gold card holders are not eligible).

How to allow us to sight your Health Care, Pension or Veteran card.

We just need to see your applicable card, so there are 3 different ways that you can share it with us:

 

- Come in to one of our Customer Service Centres and show it to a Customer Service Officer.

 

- Scan your card and email it to Outdoorhelp@merri-bek.vic.gov.au

 

- Upload a photo of your card with this form.

If you do not provide the requested information, Council may be unable to assess your eligibility or deliver the Outdoor Garden Help Program, as contractors require your contact details to schedule and complete the works.
