Concession Card (Centrelink Pension Card, Health Care, and Veteran Affairs Card accepted)

*I authorise Merri-bek City Council to use eServices to perform a Centrelink enquiry of my Centrelink or Veterans Affairs and concession card status to determine if I qualify for a payment plan.

Please note: Council cannot deduct payments from Centrelink.

Please note that for total outstanding amount:

  • Below $100, the minimum is $20 per fortnight (for Concession Card holders) and $30 per fortnight (Non Concession Card holders)
  • Between $100-$200, the minimum is $30 per fortnight (for concession) and $40 per fortnight (Non Concession Card holders)
  • Between $201-$300, the minimum is $40 per fortnight (for concession) and $50 per fortnight (Non Concession Card holders)
  • Between $301-$400, the minimum is $45 per fortnight (for concession) and $60 per fortnight (Non Concession Card holders)
  • Above $400 submit the payment plan application without the 'amount' and council will set up a minimum payment as per Council Policy

*A payment plan is a formal agreement with us and is available for a maximum of 6 months.

**Please note your instalment amount may change if your requested amount is less than the minimum required.

***Payment plans are not payable via direct debit or Centrelink deductions.

****Payment Methods:

  • Pay by Visa or MasterCard with Council’s secure online payments.
  • Mail a cheque or money order payable to ‘Merri-bek City Council’ along with your payment plan details.
  • Pay in person via cash or Visa or Mastercard at a Council Customer Service Centre. Please note: Glenroy Customer Service Centre does not take cash.

The personal information requested on this form is being collected for the purpose of assessing your eligibility for a payment plan under the Local Government Act 1989 or other relevant legislation and may also be used to update your
personal information otherwise held by Council.

The personal information may be disclosed to an appointed collections agency, Fines Victoria, Victoria Police or other Councils for matters relating to enforcement and collection of fees and may also be used by Council for related purposes.
Your information will not be disclosed to any other external party without your consent, unless required or authorised by law.
