How to join our social connections programs

People wishing to join our Social Connections and activities program are required to meet the Commonwealth Home Support Program criteria.

You can join our programs by first:

1. Contacting My Aged Care on 1800 200 422 and checking your eligibility for a face-to-face or phone assessment
2. Once you establish your eligibility you can ask for a referral to Merri-bek Social Support groups (Social Connections and activities groups)
3. Once we receive the referral we will contact you to finalise your registration
